Teresa Lynn Garrett Cook, 51, died Sunday, Sept. 29, 2013.
She was born Aug. 3, 1962, in Buffalo, Okla., daughter of Ralph and Gladys McKinney Garrett.
She lived in Hazelwood most of her life. She returned home for most of the past 10 years to be close to her family.
She loved spending time with her daughters and grandchildren.
Survivors include her parents, Ralph and Gladys of Success; three daughters, Jessee Garrett of Bucyrus, Christin Crawford of Cabool and Elishia Slattery of Rochester, N.H.; 11 grandchildren; and two sisters, Kimberly Garrett and Deanna Garrett of Success.
Services are 7 p.m. Thursday, Oct. 3, at Evans Funeral Home with Paul Murray officiating. Visitation is 6 p.m. until service time at the funeral Home.
